A personal emergency response system, or PERS, is equipment plus a response process intended to help a person summon assistance. A traditional PERS starts with a deliberate action: the user presses a help button. Some models add automatic fall detection, but providers warn that it cannot detect every fall. The fit therefore depends on both the equipment and the person’s willingness and ability to wear, charge and use it.
The three parts of a PERS device
Many traditional systems have three functional parts. They may be combined differently in a mobile unit or watch.
1. The help button
This may be a pendant, wrist button, clip, wall button, mobile unit or watch. Water resistance, battery life, charging needs, button size and automatic fall detection vary by model. Verify the exact rating and test whether the intended user can locate and press the button from the floor. If shower coverage matters, do not accept “water-resistant” without the written use instructions.
2. The base console
A home console may receive the button signal, open two-way audio and send the event through a landline, cellular connection or internet service. Stated radio range is not proof of room-by-room coverage: walls, floors and building materials matter. Test every intended room and outdoor edge. Ask how long backup power is designed to last and how connection failures are reported.
3. The monitoring center
A monitored service routes the event to a staffed center during its stated operating hours. A family-routed or autodial system contacts a preset list instead. Neither label tells you the complete process. Confirm who answers, expected answer-time reporting, two-way voice coverage, what happens without a verbal response, when emergency services may be contacted, and who maintains the contact list.
Home PERS vs mobile PERS
A home PERS works within the supported range of its base unit. A mobile PERS uses a mobile connection and may add location features. Neither label guarantees coverage; check the property, carrier, regular routes and current service area.

Choose against the person’s real week. If they regularly leave home alone, verify mobile network and location coverage along those routes. If they are usually at home, test the home unit in the bedroom, bathroom, stairs, garden and any detached space. For either type, write down who checks charging, conducts test calls and updates contacts.
The UK version: telecare and careline services
In the UK, related services are called carelines, community alarms, personal alarms or telecare. Telecare can also include linked environmental or activity sensors. So if a US-based relative mentions “a PERS” and a UK council mentions “a community alarm,” they are discussing closely related service categories. One practical UK note: GOV.UK warns that some analogue telecare devices may not work reliably on digital lines. Ask the landline and alarm providers to confirm compatibility and testing.
What a personal emergency response system costs
There is no durable category-wide price because providers change packages and separate charges differently. Ask each shortlisted provider for the same written total:
- equipment purchase or rental;
- activation, installation and shipping;
- monitoring, cellular or location service;
- automatic fall detection and additional users;
- the ordinary renewal price after any promotion;
- replacement, cancellation and equipment-return terms.

Does Medicare or Medicaid pay for a PERS?
Original Medicare does not list personal emergency response systems in its covered durable medical equipment categories. A Medicare Advantage plan may offer a separate supplemental benefit, but coverage is plan-specific. Medicaid home- and community-based services programs differ by state and eligibility group. A wearable can only help while it is worn, powered, connected and within its supported coverage. A manual alert also depends on the person being able and willing to press. Automatic fall detection adds another pathway, but major vendors explicitly say it cannot detect every fall.
